Community Transition Team
Vancouver Coastal Health
Assists mental health clients with the transition from a hospital psychiatric unit back to the community. Also provides short-term crisis-response to adults with serious and persistent mental illnesses.
Services include assistance with housing, rapid crisis response, connecting clients with community-based mental health teams, monitoring client mental health and medication as well as assistance in accessing community resources and other health services.
